Management of heart diseases has evolved significantly over last three decades and now majority of heart attack patients are being treated by coronary angioplasty thus avoiding a major open-heart surgery. However, calcium clogs (calcified blocks) in blood vessels of heart have always been a big hurdle for coronary angioplasty. Two advancements in coronary angioplasty; Rotablation & Intravascular lithotripsy make it possible to selectively debulk calcium from calcified heart vessels making them amenable to coronary angioplasty. Rotablation technique uses a diamond studded burr rotating at speed of 1.5 lakh rotations per minute to remove calcium from arteries, however it being a bit bulky may not be a feasible option in winding vessels. Intravascular lithotripsy (IVL) is the latest addition to coronary angioplasty technique which uses ultrasonic or sound waves to fragment the calcium layer in arteries making them softer. As IVL does not need any bulky burr to be used, it has less risk of complications and can be used in winding and angled vessels also.
