Parents of a 10 year old boy complained that the child had 2-3 suspected seizure episodes along with multiple episodes of sudden loss of consciousness for a brief period followed by full recovery at school. They visited a couple of doctors and then MRI brain was done for which they were told about a large cystic tumor in the child’s brain and emergency surgery was advised. Child was admitted electively a week later. Dr Swarup Verma, Chief Physician, evaluated the child and declared him fit for surgery. Dr Rahul Zamad did Endoscopic brain surgery by a small 2X3 cm craniotomy on left temporal region in form of Fenestration of the Arachnoid cyst on either side of third nerve for which general anesthesia is required , which was very well managed by Anesthetist Dr Nishant Bawankule. Post operation recovery of the patient was uneventful. 2 days later the child was discharged from the hospital. The report came out to be Arachnoid cyst and the parents were reassured that the cystic swelling was non- cancerous.
First reported in 1831 Arachnoid cyst are mostly (75%) found in male children. It is composed of fluid-filled cavity which is usually a single large cyst mostly present in the area called middle cranial fossa in the brain. Majority are found incidentally and require no surgery. Surgery is done only if pressure inside the brain increases or if the patient has other symptoms like fits, persistent headache, memory disturbances etc. Endoscopic surgery is a better way to treat this patients as post operation morbidity is minimal, recovery is very fast, infection is comparatively less than open surgery and less pain for the patients.
