60 year old female patient presented with history of difficulty in walking with pain in neck which was moderate in intensity with history of weakness over left side of the body followed by right side which had increased over last 2 weeks. She had weakness in lower and upper limbs. She could not work and do her daily routine properly. She was evaluated by Dr. Rahul Zamad and he suggested MRI. MRI revealed significant osteolysis of the C4-C5 vertebral bodies with cord compression. Chest X-ray did not show any significant changes suggestive of tubercular disease.
She was admitted to the hospital as a case of POTTS spine and her pre operation performance status was worked upon. She was evaluated further with no features of tuberculosis anywhere else in the body. Once her overall condition improved she underwent surgery where her c4 and c5 vertebral bodies were completely removed to relieve the compression over the spinal cord and it was replaced by titanium cage fixed with screws which became patients new vertebral bodies.

After the surgery her pain was reduced and weakness slowly improved and she was discharged after 7 days of surgery. Diagnosis was confirmed based on Gene expert report which is sensitive for tuberculosis. She was started on antitubercular treatment as per physician advice.

Dr. Rahul Zamad said , ” Potts spine or tubercular spondylodiscitis is still a very common disease in country like India due to unfavorable socioeconomic conditions and it remains a burden on the health sector. Thoracic and lumbar region are most common location. Cervical spine involvement is seen in only 2-3 % cases. Cervical canal is narrower as compared to thoracic and lumbar and hence chances of neurological deficit are more in this patients and hence it warrants an early intervention. Surgery is treatment of choice if spinal cord compression is present. In patients with diabetes blood sugar has to be strictly controlled for healing of the wound. Antitubercular treatment in this patients has to be given for a minimum of 1 year. Post op rehabilitation in form of physiotherapy plays important role in recovery of weakness and further strengthening the muscle power. “
