A 70 years old male patient presented with complaints of multiple episodes sudden loss of consciousness for a brief period followed by complete recovery. He came to Wockhardt Hospitals Nagpur. His MR cerebral and neck angiography showed severe narrowing in left Carotid artery. He underwent left Carotid Endarterectomy. The patient post operatively did not show any evidence of stroke and angiography showed good flow across left carotid artery.
In another case a 67 year old male patient presented with history 2 episodes of sudden clumsiness over left upper limb which was persisting but he was able to carry out his daily activities. Evaluated with MRI followed by 4 vessel DSA which showed severe stenosis in bilateral carotid arteries. Right Carotid Endarterectomy was done in view of being the symptomatic side. Post operation patient did well with subtle weakness over left side which improved slowly.
Dr. Rahul Zamad informed, these are two different patients, one has stenosis on one side and the other patient has on the other side. Carotid artery disease causes up to one-third of all strokes. A stroke occurs when something blocks blood flow to your brain, causing brain injury.
Dr. Rahul Zamad said , “Carotid Endarterectomy is a surgical procedure to remove a build – up of fatty deposits (plaque), which causes narrowing of a carotid artery. The carotid arteries are the main blood vessels that supply blood to the neck, face and brain and if carotid artery becomes blocked or a blood clot forms, a piece may break off and go to the brain which can result in a stroke or a transient ischaemic attack which is sometimes known as mini stroke( is caused by a temporary disruption in the blood supply to part of the brain) from which recovery is complete usually within 24hours.”
Dr Zamad further added , the procedure significantly reduce the risk of a stroke in people with severely narrowed carotid arteries. He mentioned that symptoms of carotid stenosis can be weakness or numbness of face, arm or leg, speech problem or loss of vision in one eye. The operation should be carried out as soon as possible after symptoms appear or electively if carotid narrowing is incidentally detected.
Both the patients recovered at the hospital and were discharged.
He explained, in this procedure, incision is taken over on the front of the neck and the procedure may be carried out even with patient being completely awake. After the procedure patient is observed for 2 to 3 days before being discharged. Post operation Angiography is done at a later date. Endarterectomy should be first line of treatment for most patients. Carotid stenting is alternative to this procedure and is less invasive but may be associated with increased chances of periprocedural stroke.
