One 76 Year Old gentleman from Nagpur and another 63 year old lady from Chandrapur came to Wockhardt Hospital, Nagpur. The Male patient had a problem of Diabetes, Hypertension, Heart Attack and Old Stroke and Female patient had a problem of Diabetes, Hypertension, Heart Failure and Kidney disease. Both patients were seen by Dr. Nitin Tiwari, Sr. Interventional Cardiologist, Wockhardt Hospitals, Nagpur and he found lots of calcium in their coronaries.
Coronary calcium is the Achilles heel in coronary angioplasty. The calcification of coronary arteries leads to suboptimal stent expansion which can lead to reblockages and clotting in the stents. Calcification of coronaries can be due to aging, kidney disease, diabetes, etc. It hampers the chances of successful angioplasty and crossing and re-expansion of stents. Rotablation is a technique in which a diamond burr is passed into the calcified coronaries and the arteries are drilled at around 2 lac rotations per minute to break the calcium into fine particles.
IVL (Shockwave intravascular lithotripsy) is a recently approved technique in which through a balloon lithotripsy is delivered by vaporising fluid to create an expanding bubble which cracks the calcium. Similar technique is used to break kidney stones.
Dr. Nitin Tiwari treated both patients,one 76 years old(Nagpur) and other 63 years old(Chandrapur) by combining the above two techniques Rotablation and Lithotripsy (Rotatripsy).Two consecutive cases were done within two days, the first time ever in Central India. In both the cases the balloons were not crossing the calcified coronaries,so IVL balloon could not cross, hence Rotablation was done and the internal calcification was removed. The external wall calcification was broken by IVL.Hence both the complemetary techniques were used in the patients. Then regular angioplasty with stenting was done and both the patients were discharged uneventfully.
