American Oncology Institute (AOI) at Nangia Specialty Hospital in Nagpur, achieved a significant milestone in the successful treatment of a rare case of plastic bronchitis in a 15-year-old boy who presented with chronic breathlessness and fever. This challenging case was managed by Dr Deepak Muthreja, Head of the Pulmonology department at AOI.
The patient’s medical history revealed a prior episode of pneumothorax four years ago, which was locally treated. Upon examination, a CT scan indicated a right-sided loculated effusion with collapse of the right lower lobe. To investigate further, the patient underwent a bronchoscopy, revealing the presence of thick inspissated mucus, characteristic of plastic bronchitis. Unfortunately, post-mucus removal, the patient developed Acute Respiratory Distress Syndrome (ARDS) necessitating immediate mechanical ventilation.
Dr Deepak, reflecting on the case, stated, “The development of ARDS following the removal of mucus posed a significant challenge. The patient’s condition was further complicated by a resistant pseudomonas infection, requiring invasive ventilation and repeated bronchoscopies for effective treatment.”
The medical team’s tireless efforts, coupled with advanced medical interventions and support, led to a remarkable recovery for the patient. After 14 days of intensive care and treatment, including bronchial toileting and specific antimicrobial therapy, the patient responded positively and was discharged with home oxygen support. It is noteworthy that the patient, who initially required oxygen support, presently does not need any additional respiratory assistance.

Plastic bronchitis is an uncommon condition in which the lymph fluid builds in the airways and forms rubbery plugs, known as casts, which block the airways and make it difficult to breathe. Plastic bronchitis can be extremely debilitating and in some rare cases life threatening. Plastic bronchitis is associated with congenital heart disease in children (such as single ventricle defects) and diseases of the lymphatic system in adults.
