17 Year Old Boy, only child, presented with history of trauma due to head on collision with a car late night at one of the prime locations in Nagpur. When he was evaluated by Dr.Rahul Zamad,Consultant- Neurosurgery(Brain and Spine surgeon) at Wockhardt Hospitals-Nagpur, he had altered sensorium and was very irritable. An urgent CT scan of the brain was done which showed a large area of bleeding in the brain just above the eye on both sides with right side much more than left.
Dr.Rahul Zamad,Consultant- Neurosurgery(Brain and Spine surgeon) at Wockhardt Hospitals-Nagpur,said that, we counselled the relatives and told them regarding the seriousness of the situation and need for surgical intervention as soon this bleeding would start showing its pressure effect and it would be a lifesaving surgery. As he was a young chap and we wanted to avoid large incisions we explained relative about minimally invasive approach for this bleed which is well described in the literature with chances of needed bigger surgery if required which they agreed upon. We made a small incision over the right eye brow and removed a small area of bone above the right eye following which the blood was evacuated which was under tremendous pressure. After removal of the blood the brain was relaxed and good pulsations were seen. Meticulous care was taken that there is no fresh bleeding present inside the brain while doing closure. The child was observed in ICU for 2 to 3 days and then in the ward for another 3 day before being discharged. On follow up he was doing fine with some issues with right eye and vision and occasional irritability which was acceptable.
