30-year-old male worker from Kamptee was referred with 65 % burns with b grade 3 mixed burns. The cause of burns in this case was splash of hot metal liquid of 1000° F while on duty. Immediately after the burns, due to severe pain he rushed to the nearby pond and sat there for 10 mins The patient was initially given first aid before he was transferred to Orange City Hospital.
It is extremely important to get the patient’s immediate medical attention in case of burns injury as the patent needs to be rehydrated, vitals monitored and various infection control protocols that need to be addressed at the earliest. Skin being the largest organ of our body; when it is exposed in case of burns the chances of acquiring infection is significantly higher than usual.Thus, making it extremely critical to follow infection control protocols for the burns patent.
Renowned plastic surgeon with expertise in burns, Dr. Darshan Rewanwar has been associated with Orange City Hospital for 20 plus years. He attended the patient immediately in the emergency room and transferred to an isolation unit with constant monitoring facilities.
After initial stabilization, he was thoroughly examined and further testing revealed that the patient was positive for Hepatitis B surface antigen (HBsAg) which was another challenge for the treating surgeon and the entire team considering it is contagious.
Once the patient was stabilized and after obtaining consent from the relatives, the patient underwent series of surgical procedures escharectomy , sequential debridement and burn dressings, VAC therapy, mega skin grafting, graft and donor site dressings with proper splintage.
