A 64-year-old male patient was brought to the Emergency & Trauma Care Department at Alexis Hospital by relatives and neighbours with history of severe chest pain and profuse sweating along with loss of consciousness at home . On examination by Dr. Rupesh Bokade, Emergency & Critical Care physician, his ECG was suggestive of a massive heart attack. On further examination it was observed that the patient had very low blood pressure, heart rate was continuously dropping from 30/min to as low as 7/min and he also had severe breathing difficulty. To counter this, we immediately started percutaneous cardiac pacing (to increase heart rate), intubated the patient and put him on a ventilator within minutes of his arrival in our emergency department.
After carrying out the life saving measures in the Emergency department, the patient was immediately shifted to Cath Lab where Dr. Shoeb Nadeem, Interventional Cardiologist at Alexis , performed coronary angiography followed by temporary pacemaker implantation and primary angioplasty (stent placement to remove block in the blood vessel of heart)… Immediate relief with normalization of heart rate and gradual improvement in his blood pressure was noted. He was successfully weaned off ventilator on the next day itself and discharged after two days of continuous monitoring, treatment & observation.
