56-year-old woman was transferred from another hospital to Orange City Hospital as she suddenly went unconscious. The patient was immediately resuscitated by the Intensive Care Team and after a thorough examination and investigations, it was revealed in CT SCAN that there was bleeding in different parts of the brain. The patient was placed on a mechanical ventilator and given inotropic support and admitted under Dr. Vishal Tarale, Team Orange Neurosurgeon. Radiologist Dr. Mayur Mahadule reported Diffuse SAH ( Sub-Arachnoid Hemorrhage), intraventricular hemorrhage, SDH (Sub-Dural Hemorrhage) and intraparenchymal hemorrhage was seen with suspected ACOM (Anterior Communicating Artery Aneurysm) of the brain – the most complex in all cerebral aneurysms. Aneurysm happens when an area in an artery weakens and balloons.
Aneurysms occur in bigger vessels supplying the brain, back of the knee, intestine or organs. A ruptured aneurysm can result in internal bleeding and stroke. It can be fatal and needs immediate attention. The symptoms are sudden and include severe headache, including nausea and vomiting, stiff neck, blurred or double vision, seizure, loss of consciousness and confusion.
Dr. Shishir Rawekar, Interventional Radiologist needed to do the preliminary DSA (Digital Subtraction Angiography) which showed evidence of a ruptured saccular aneurysm arising from the ACOM, which required immediate intervention to stop further bleeding and save patient’s life. Dr. Shishir performed one of the most challenging, complex, intricate procedures of performing an endovascular embolization of the ACOM aneurysm with coiling at the CATH LAB successfully.
Dr. Neeta Deshpande extended Anaesthetic support for the hi-risk case as the patient had several underlying co-morbidities like hypertension, diabetes and obesity which made the management precarious. Follow up CT scan brain showed no further bleeding and patient stabilized. Patient improved during the course of treatment at the hospital under the expertise of Dr. Rajesh Atal who is heading Critical Care Unit at the hospital.
