Recently, a middle-aged woman reported to 1020 bedded Shalinitai Meghe Hospital & Research Centre (SMHRC), Wanadongri, Nagpur, after enduring months of suffering from recurrent cholangitis. She had a history of open cholecystectomy with common bile duct (CBD) exploration and subsequently developed a persistent distal CBD stricture. Despite undergoing multiple ERCP procedures, CBD clearances, stenting, and biliary drainage at an outside hospital, she failed to obtain lasting relief. Her jaundice progressively worsened, appetite declined, and repeated hospital visits severely affected her physical strength and emotional well-being. Upon arrival at SMHRC, the Department of Surgery conducted a comprehensive clinical and radiological evaluation to identify the underlying cause of recurrent infections and biliary obstruction. Dr. Rajiv Sonarkar, HoD Surgery informed that, “After detailed multidisciplinary deliberations, the team concluded that a definitive surgical intervention was essential to prevent further complications and restore quality of life. Choledochoduodenostomy, a surgical bypass connecting the common bile duct directly to the duodenum for unobstructed bile drainage, was planned.” Accordingly, the patient’s relatives were counseled in detail and, after understanding the condition and proposed management, they consented for surgery with full confidence in the treating team.
This achievement was made possible through the dedicated efforts of the Department of Surgery, led by Dr. Sudhir Tomey, Senior GI Surgeon & Adjunct Faculty, with the support of Dr. Rajiv Sonarkar (HOD Surgery), Dr. Aarti Kirange (GI Surgeon), Dr. Vandana Tomey, Dr. Danish, Dr. Ashish, and Dr. Rupesh, along with junior residents Dr. Sandeep, Dr. Gaurav, Dr. Simran, and Dr. Amev. Seamless perioperative care was ensured by the Anaesthesiology team comprising Dr. Deshmukh, Dr. Anjali Modak, Dr. Ranjith, Dr. Shivani, and JR Dr. Joseph.
The procedure was successfully performed free of cost under the MJPJAY scheme, marking a significant turning point in the patient’s prolonged medical journey. Postoperatively, the patient showed rapid clinical improvement. Her jaundice subsided, infection parameters normalized, and her strength and appetite gradually returned. Despite the complexity of the surgery, she was managed entirely in the ward with meticulous monitoring, reflecting the hospital’s high standards of postoperative care. She recovered smoothly without complications and was discharged in a stable condition, finally experiencing relief after months of distress.
