A 30 year old police officer from Madhya Pradesh posted in Umrakhed village in Maharashtra for election duty suddenly fell ill with fever and body ache. He was admitted to Yavatmal from where he was referred to Orange City Hospital owing to detiorating health.
The young man was found to be Dengue NS1 positive with low platelets count at the time of admission. On comprehensive testing, the ECG showed typical features of Acute Coronary Syndrome (ACS) – a term used to collectively describe conditions resulting from reduced blood flow to the heart. Interestingly, he did not have any symptoms of heart attack, like chest pain at the time. Echocardiography confirmed the effect on the heart, and the opinion of cardiologist-Dr Amar Aamle was immediately sought.
The challenging aspect of treatment was administering blood clot therapies and anti platelet medications for the ACS, while platelet counts were already low due to Dengue.
Dengue fever was initally managed conservatively, and platelet count showed a steady rise. The decision to do coronary Angiography (CAG) was then taken with caution in view of the increased risk of bleeding complications. The CAG revealed a blood clot in one of the vessels of the heart, and it was decided to proceed with medical management.
The young man made a full recovery and was discharged with normal platelet count.
This case was a classic example where early intervention was possible because of early diagnosis considering the multiusciplinary approach that was needed for this patient. Cardiology unit along with critical care unit had to work in tandem to get such outcome.
